Understanding the Vestibular System
The vestibular system — located in the inner ear — is your body's primary balance and spatial orientation system. It detects head movements and position changes, and sends this information to the brain to coordinate eye movements and maintain balance. When the vestibular system is damaged, misfiring, or compensating for injury, the result is dizziness, vertigo, or imbalance.
BPPV: The Most Common Cause of Vertigo
Benign Paroxysmal Positional Vertigo (BPPV) is by far the most common cause of vertigo, accounting for approximately 50% of cases. It occurs when tiny calcium carbonate crystals called otoconia ("ear rocks") that normally sit in a fluid-filled chamber of the inner ear (the utricle) become dislodged and migrate into one of the three semicircular canals. When you move your head, these displaced crystals move within the canal, causing abnormal fluid movement and the sensation of violent spinning.
BPPV is typically characterised by:
- Brief episodes of vertigo (lasting 30-60 seconds) triggered by specific head positions
- Most commonly triggered by lying down, rolling over in bed, looking up, or bending forward
- Associated nystagmus (involuntary eye movement) that Dr. Nidhi can observe during assessment
The Epley Manoeuvre: Often Curative in a Single Session
BPPV can often be cured — completely and rapidly — with a simple repositioning manoeuvre called the Epley Manoeuvre (or Canalith Repositioning Procedure). This involves guiding the patient through a series of precise head and body positions that move the displaced crystals back to where they belong. Many patients feel immediate improvement after a single session. This is genuinely one of the most satisfying treatments in all of physiotherapy — a 20-minute procedure that gives someone their life back after months of debilitating dizziness.
Other Causes of Dizziness and Imbalance
Vestibular Neuritis and Labyrinthitis
Often following a viral illness, these conditions cause inflammation of the vestibular nerve or inner ear structures. The result is usually sudden, severe vertigo followed by a period of dizziness and imbalance that can last weeks to months. Vestibular rehabilitation — a specific programme of exercises that help the brain compensate for the reduced vestibular input — is the primary treatment.
Meniere's Disease
Characterised by episodic vertigo, fluctuating hearing loss, tinnitus, and ear fullness, Meniere's Disease is a complex condition. Physiotherapy focuses on vestibular rehabilitation between attacks, fall prevention, and helping patients recognise and manage triggers.
Cervicogenic Dizziness
Neck disorders — including cervical spondylosis, whiplash, and upper cervical joint dysfunction — can cause dizziness and imbalance by disrupting the proprioceptive signals from the neck that contribute to the brain's balance calculations. This type of dizziness is often associated with neck pain and stiffness, and responds well to cervical manual therapy and specific neck exercises.
Central Causes
In a minority of cases, dizziness is caused by central nervous system problems (migraines, stroke, multiple sclerosis). These are assessed during Dr. Nidhi's comprehensive assessment, and patients are referred appropriately if central pathology is suspected.
What Vestibular Physiotherapy Involves
- Detailed assessment including standardised tests of balance, gait, and eye movement (VOR assessment)
- Hallpike and Roll tests to determine the type and canal affected in BPPV
