YouTube stretching videos. Generic "10 exercises for back pain" articles. Apps that tell you to foam roll everything. The internet is overflowing with physiotherapy content — and some of it genuinely helps people. But a significant amount of it is oversimplified, unqualified, or simply wrong for your specific situation. And the line between "home physiotherapy that helps" and "home physiotherapy that sets you back" is not always obvious.

What Actually Works at Home

1. Low-Load, Consistent Movement

For almost all musculoskeletal conditions — back pain, neck pain, joint pain, post-surgical recovery — regular gentle movement is beneficial. This does not mean aggressive exercise or pushing through pain. It means walking, gentle cycling, swimming (if accessible), and staying active within your pain tolerance. The body heals better when it moves.

2. Heat and Ice (Used Correctly)

Ice is effective for acute injuries in the first 48-72 hours — it reduces swelling and numbs pain. Apply for 15-20 minutes, wrapped in a cloth, not directly on skin.
Heat is better for chronic muscle tightness and joint stiffness — it increases blood flow and relaxes muscle spasm. Use for 15-20 minutes before gentle movement.
What doesn't work: Using ice for chronic pain (it can reduce the beneficial inflammation involved in healing) or using heat on an acute injury (can increase swelling).

3. Prescribed Home Exercise Programmes

Exercises given to you by a qualified physiotherapist after a proper assessment are among the most effective things you can do at home. The key word is "prescribed" — exercises tailored specifically to your diagnosis, your movement impairments, and your stage of recovery.

4. Posture Breaks

Changing position regularly — every 30-45 minutes for desk workers — is genuinely therapeutic. Set a timer. Get up, walk 10 steps, do a few shoulder rolls or a standing back extension. This one habit can prevent and relieve a large proportion of office-related musculoskeletal problems.

5. Sleep Positioning

How you sleep has a significant impact on neck, back, and hip pain. Sleeping on your back with a pillow under your knees (for low back pain) or on your side with a pillow between your knees (for hip or sciatic pain) can dramatically reduce overnight pain and morning stiffness.


What Doesn't Work — or Can Harm

1. Aggressive Stretching of Acutely Injured Structures

Stretching a freshly torn or inflamed muscle or tendon is counterproductive and can worsen the injury. Acute injuries need protection, not aggressive lengthening. This is one of the most common mistakes people make.

2. Random YouTube Exercise Programmes Without Assessment

An exercise that helps one person's back pain can make another person's back pain significantly worse — because the causes are different. A disc herniation requires different exercises from a facet joint problem, which requires different exercises from sacroiliac joint dysfunction. Without knowing your diagnosis, generic exercise programmes are a gamble.

3. Over-Relying on Foam Rolling

Foam rolling provides temporary relief of muscle tension — it can feel wonderful — but it does not address the underlying cause of why those muscles are tight. If your hamstrings are perpetually tight because of a hip or lumbar problem, rolling them 10 times a day will give you temporary relief but not resolution.

4. Pushing Through Sharp or Worsening Pain

The "no pain no gain" mantra has no place in physiotherapy. Sharp pain, pain that radiates, pain that is significantly worsening with exercise, or pain accompanied by neurological symptoms (numbness, tingling, weakness) are signals to stop and seek professional assessment.

  • Pain that has not improved after 2 weeks of home management
  • Pain that is getting progressively worse despite rest and self-care
  • Nerve symptoms (numbness, tingling, or weakness) in a limb
  • Pain following a significant trauma (fall, accident, sports injury)
  • Night pain that consistently wakes you from sleep
  • Pain associated with unexplained weight loss, fever, or bowel/bladder changes (see a doctor urgently)
