Weeks 2-6: Strengthening Begins

  • Progressive hip abductor strengthening (side-lying leg raises, standing hip abduction)
  • Quadriceps and gluteal strengthening
  • Balance and proprioception training
  • Gait retraining — eliminating the Trendelenburg limp that is common after THR
  • Gradual reduction of walking aids

Weeks 6-12: Function Restoration

  • Returning to driving (if cleared by surgeon)
  • Stairs without handrail
  • Step training and uneven surfaces
  • Cardiovascular fitness recovery
  • Return to recreational walking, swimming, cycling

Months 3-6: Return to Full Activity

Most hip replacement patients can return to virtually all daily activities and many recreational pursuits by 3-6 months with the right rehabilitation. High-impact activities (running, jumping) are generally discouraged to protect implant longevity.
